African-American Rhinoplasty is a type of ethnic nose job or reconstructive nasal surgery to change the shape of those whose ancestry draws back to Africa with unique facial recognition.
Individuals who’s ancestry comes from Africa typically have very dark skin because of their climate and location close to the equator, where the sun shines strongest. This is taken into consideration when planning the operation. For example, If an external incision is to be made, might there be darkening as the incision becomes a scar?
Certain features are in need of change in each patient but not all features need correction. From the standpoint of planning, computer imaging is very helpful, because African-Americans having rhinoplasty do not want nor expect the nose of a similarly aged Irish-American.
We have had long experience with African-American patients. As Major Kotler, US Army Medical Corps, Dr. Kotler has treated American servicemen of all colors and ethnicities. Owing to the mixed genetics which continues to increase due to immigration and intermarriage, there is a wide variety of nose sizes and shapes of African -American noses. There is not one kind, no one shape, no one skin thickness or color. Nostril shape and size is variable. So is the height of the bridge and the width of the upper and lower parts.
There are some features that are a bit more prevalent, however. This would include wider nostril base, thicker wings or alae, softer tip cartilage, flatter tip and less forward projection of the nose.
The results that are possible after an African-American Nose Job are varied. Some will require cartilage grafting or, after surgical healing is complete, three to four months after surgery, permanent non-surgical rhinoplasty in a few spots. Such as to give the tip a bit more projection and height. But in the end, as long as the operation yields natural results, the patients will be happy. Major changes where the patient looks too “different” are less desirable.
African-American rhinoplasty cost can vary. The factors in determining the rhinoplasty fee are as follows:
The most specialized and sophisticated, hyper-experienced surgeons generally have higher fees in respect for their talent. However, generally, due to their skill they perform the operation in less time and that can shorten the healing time and interval away from work. A faster nose recovery has great value.
Costs for the surgical facility and anesthesia service need to be considered. These are the two other components of the global or total fee. Highly specialized, licensed surgical facilities have the same safety standards as a hospital. Their fees may be higher. But that is the price of quality and most importantly, safety.
Having a doctor-anesthesiologist may cost a bit more than having the surgeon be responsible for the anesthetic or having a Certified Registered Nurse Anesthetist at the controls, but again, the doctor-anesthesiologist brings the longest training and highest degree of specialization to the patient.
Shopping for an African-American Rhinoplasty by price has some hazards, obviously. The cost of dissatisfaction will be more than just dollars. Or, as one patient’s husband, a Critical Care Unit RN noted: “If you think the first rhinoplasty is expensive, wait until you have to pay for the second, or even third. You want to do it right the first time”.
Rhinoplasty operative time for an African-American nose will be just under two hours. If internal nasal surgery to improve breathing or improve sinus function is done at the same session, total time in the OR will be two and half hours.
We have many before and after photos of our African-American Rhinoplasty patients. These photos show how there is no one “template” or one “formula” for a successful outcome. The operation must be customized.
The surgical exercise is the same for the African-American Rhinoplasty as it would be for any other rhinoplasty. Often, however, the flat, thick tip requires more attention. Ethnic procedures during surgery that might be employed are grafting a piece of cartilage taken from the nasal interior such that the tip will be stronger and less flat and receding. Also, fat may have to be removed from the tip and that is done by withdrawing cartilage, via internal incisions, from the nose, “ working on the tip cartilage, including shaving off the fat and trimming excess cartilage and then returning the reshaped cartilages back into their natural bed within the nose.
Risks are no greater for the African-American Rhinoplasty patient than any other patient. However, sound medical practice demands that all aspects of the patient’s general health be evaluated before surgery. African-Americans are subject to a genetic disorder, Sickle Cell Anemia, whereby the red cells are abnormal and consequences can occur in certain situations that would not be desirable. But all such issues are sorted out well before surgery and include lab tests and consultation with the patient’s doctor.
African-American Revision Rhinoplasty is not totally unusual for our practice. We see patients dissatisfied with their appearance and sometimes also the breathing after their prior rhinoplasty elsewhere. Usually, they can be helped. It may require a combination of a surgical revision rhinoplasty and permanent non-surgical revision rhinoplasty to give the patient a better and natural nose that breathes well.
